In a marine context the act of investigation can be doubly complex given the event is often at sea, in another part of the World and likely moving.
Organisations need to have the capability and capacity to proportionally respond to incidents which align to the Actual or Potential of the incident.
An investigation needs to be meaningful and by that it needs to produce the factual based and ethically obtained data which clearly identifies the systemic reasons.
